From Pharmacy Drug Guide
Topamax® patient assistance programs help qualified patients with free medication for up to a year. Patient assistance programs, also known as PAPs, provide patients experiencing financial difficulties with the opportunity to receive their medication for free or at a discounted cost.
The main patient assistance program available for Topamax® patients is offered through the Johnson & Johnson Patient Assistance Foundation. Patients struggling to afford medication can review eligibility requirements for patient assistance for Topamax®, as well as other savings opportunities available for the medication. In addition to Johnson & Johnson, several other pharmacies and organizations offer free or discounted Topamax® to eligible patients.

Patient Assistance Program Overview
Topamax Patient Assistance Programs
The primary patient assistance program available for Topamax®, operated by Johnson & Johnson, offers free Topamax® to qualified individuals for up to a year, and is only provided to patients who have no other source of prescription drug coverage available to them.
Eligibility for the Topamax® patient assistant program is based on the following:
- Patients must reside in the United States, Puerto Rico, or the U.S. Virgin Islands
- Patients must be treated by a licensed U.S. healthcare provider
- Patient must not have prescription insurance coverage
- Must have an annual income of $21,660 or less for individuals and $29,140 or less for a family of two. Income requirements for larger families are based on a sliding scale.
- For physician-administered drugs, the allowable income rate is higher, at $43,320 or less for singles, $58,280 for couples and more for larger families.
In order to enroll in the program, patients can either download the application online through the foundation's official website, http://www.jjpaf.org/eligibility/index.html, or contact a foundation representative by calling 800-652-6227. Patients contacting the foundation by phone will receive the application by fax or by mail.
Once a patient has downloaded or received the application, they must fill it out and gather all the required documents, including a copy of the patient's most recent federal tax return. The patient must then take the application and all documents to their healthcare provider to complete. The provider will then need to fax the patient's completed application to 888-526-5168, or mail the application to:

Patients who do not qualify for the Johnson & Johnson program might still find aid from Rx Outreach medications. This nonprofit organization offers Topamax® at a reduced price for eligible patients, charging $35 for a 90-day supply and $65 for a 180-day supply. The Together Rx Access Card is another option open to those with no insurance and an income of $45,000 or less for singles and a sliding scale for larger families.
